MSHSAA MEDICAL ELIGIBILITY FORM
- Form Link: MSHSAA Medical Eligibility Form
- THIS FORM MUST BE FILLED OUT BY YOUR DOCTOR when students get their physical exam completed. (This form is required to be uploaded to your PRIVIT account.)
- The physical exam date acceptable for the 2024-2025 school year is July 1, 2023 or after. Physical exams dated July 1, 2023 or after will be eligible for two full school years. Since July 1st is a firm cut off each year, it is in your child’s best interest to have the physical examination completed after July 1st. (Physical exam forms will no longer be collected or uploaded to your PRIVIT account. Only the MSHSAA Medical Eligibility Form)
PRIVIT (Create a new account or update an existing account)
- PRIVIT is an online activity registration. PRIVIT streamlines the tryout process by eliminating the need for paper forms.
- Each year (after July 1), every student involved in any MSHSAA School Sponsored athletic/activity program needs to review their account to ensure the most up to date information is listed, make changes as needed, and electronically sign the information required.
- Parents, coaches, sponsors, and administration can download the PRIVIT app onto their phones to access the information. This will make it more efficient to easily access emergency information at both home and away events.
